3. If your Number, does not match your name.

 

4. You must Complete this Form.

5. Providing ID is OPTIONAL.

    HOWEVER: If your number, is not registered to you; then      you must provide ID.

  

6. If you do not feel comfortable, with providing ID. Then           you can Email me, using your work email; instead

Hours: Monday through Saturday 10am - 7pm

             Sunday I am off.

             Last Minute appointment, is possible;
